By the time you’re looking at the medical certificate, you’ve probably already spent weeks researching income thresholds, health insurance, and BLS appointments. The last thing you need is to get this part wrong.

The good news is that the medical certificate is one of the more straightforward documents in your NLV application — once you know what it needs to say and where to get it. Do NLV Applicants Need a Medical Certificate?

Yes — it’s a mandatory part of the application. The Spanish consulate requires a medical certificate of good health as standard for all Non-Lucrative Visa applicants, regardless of age or health status. Without it, your application will be rejected before anything else is assessed.

What Must It Include?

Does It Need an Apostille?

Yes, always. The apostille is the official stamp from the FCDO that makes your UK-issued certificate legally recognised in Spain. Without it, the consulate will not accept the document — full stop.

Does It Need to Be Translated into Spanish?

Yes, if issued in English. It must be a sworn translation — carried out by a translator officially registered with the Spanish government. An informal translation or one done by a bilingual friend won’t be accepted.

What About Dependants?

This is one of the most commonly overlooked aspects of the NLV medical certificate process. If you’re bringing a spouse, partner, or dependent children as part of your application, each of them will need their own medical certificate — issued separately, apostilled separately, and translated separately.

It’s not enough to have one certificate that covers the whole family. Spanish consulates require individual certificates for each applicant, including children. The requirements are the same for dependants as they are for the main applicant — same wording, same format, same doctor.

What About Renewal?

The NLV is initially granted for one year. When you apply to renew — for a further two years — you will need a fresh medical certificate. The original certificate you submitted with your initial application will have long since expired by the time your renewal comes around.

The renewal certificate follows exactly the same requirements as the original — same wording, same format, same apostille and translation requirements. The difference is that renewal applications are submitted in Spain rather than at a UK consulate, so you’ll need to check the specific requirements of your local Spanish immigration office (Oficina de Extranjeros) at the time of renewal.

Worth planning for: don’t assume the certificate from your initial application carries over. It doesn’t.

Timing — When Should You Get It?

NLV applicants often have more complex timing considerations than other visa types. You may be coordinating the sale of a property, a retirement date, a tenancy end, or a school term — all of which create fixed points around which the application has to fit.

A few things to factor in:

The certificate is valid for three months. It must have been issued within the 90 days before your consulate appointment — not your planned move date. If you get it too early it may expire before your BLS appointment.

BLS appointment slots are limited. Especially in spring and early summer, when NLV applications tend to spike. Book your BLS appointment as early as you can, then work backwards from that date to figure out when to get your certificate.

Allow time for apostille and translation. If you’re not using a combined service, factor in at least one to two weeks for the apostille to come back from the FCDO, plus translation time on top of that.

If applying as a family, multiply everything. Multiple certificates, multiple apostilles, multiple translations — all need to be coordinated so that everything arrives together in time for your appointment.

As a general rule, aim to get your medical certificate — and those of any dependants — four to six weeks before your BLS appointment.

Can You Get It Through Your GP?

Possible in some cases, but rarely the smoothest route. Most NHS GPs don’t offer this service, and those who do are often not FCDO-registered — which creates a notarisation step that adds cost and time you probably don’t want when you’re already juggling a complex application.
