A properly-formatted Spanish visa medical certificate has several specific elements. Here’s what to look for on yours.
At the top of the certificate, you should see the issuing medical center’s letterhead or logo, along with a clear title identifying the document. In this example, “Visa Medical Certificate of Good Health” makes it immediately clear what the document is. Spanish consulates expect to see this on official medical center stationery, not a plain page.
The body of the certificate begins with the English wording. It identifies the applicant by name and states that they don’t suffer from any disease that could cause serious repercussions to public health, in line with the International Health Regulations of 2005.
The IHR 2005 reference is the part that makes the certificate work. A doctor’s letter saying “this patient is in good health and fit to travel” won’t be accepted, regardless of how well it’s signed. The specific reference to the International Health Regulations is what Spanish consulates are looking for.
Below the English wording, the same statement appears in Spanish under the heading “Certificado Médico de Buena Salud.” Having both languages on the same document removes the need for a separate sworn translation, saving cost and time. The bilingual format used by US Spanish consulates follows a consistent set of templates which are available to view.
If your certificate is in English only, you’ll need to arrange a sworn translation by a translator registered with the Spanish Ministry of Foreign Affairs. The bilingual format is accepted by US Spanish consulates.
In the lower section of the certificate, you should see the medical center’s official stamp. This confirms the authenticity of the issuing clinic. In the example, Wilmer Health’s clinic stamp appears in the dedicated box on the left.
The signature must be a wet ink, hand-signed signature from the doctor who issued the certificate. Not a digital signature, not a printed signature, not a stamp facsimile. The example shows the doctor’s hand-signed signature in the center box.
If you receive a certificate where the signature looks printed or digital, the consulate may reject it.
Spanish consulates require the doctor’s license number on the certificate, as proof of credentials. In the example, this appears in the right-hand box, showing the doctor’s name and license number clearly.
At the bottom of the certificate, you should see where and when it was signed. In this example, the certificate was issued in New York City, USA, on 7 July 2025.
Date matters because Spanish consulates require certificates to be issued within 90 days of your visa appointment. Anything older won’t be accepted.
